The collaborative efforts between Catholic Charities and Santa Rosa Community Health (SRCH) have significantly enhanced health care accessibility for Sonoma County’s homeless population by co-locating services which reduce emergency room visits and improve care coordination. Programs like CalAIM have further supported these efforts by providing critical case management and funding. Policy recommendations suggest that extending capacity-building funds and expanding billing options will better support vulnerable populations. Together, these initiatives are transforming health and housing outcomes for individuals experiencing homelessness.
